Coach form- Sign up as a head coach and receive a $25 discount. Assistant coaches receive a $10 discount.

Referee Game Report- Referee Game Report. This form must be completed and turned in to EYSA Administration in order for Referee pay to be granted.
Sponsors Form- Opportunity to help our children grow! Sign up to sponsor a team and advertise your business! EYSA is a not for profit organization. Each year EYSA struggles to find enough sponsors for all of our youth soccer teams. Sponsors will receive a dated plaque with a team picture, as well as have the business name and logo on team jerseys and a link on our webpage!
