This Form is to Sponsor one of our divisions of players; 5U, 7U, 9U and 12U.  This is based on availability of space on the jerseys.  We have options for the back of the jersey or on a sleeve.  

This form is to sponsor our entire leage of players.  We are limited to one League Sponsor and the Name/Logo goes on the back of all of the jerseys.
